I hereby authorize The Playhouse Company to forward my personal information to verification suppliers acting on behalf of The Playhouse Company.

I authorize The Playhouse Company suppliers to furnish personal information regarding my credentials to The Playhouse Company. I unconditionally indemnify The Playhouse Company and its verification information suppliers against any liability which results or may result from furnishing information in this regard.
